Community Hub Team
Most people with a life limiting illness wish to remain in their own homes and our Community Hub services are here to help you do that.
Community Hub Team provides a service for patients, families or carers, at home or in the Inpatient Unit. This could be at the time of diagnosis or at any point throughout an illness.
They undertake a comprehensive nursing assessment to find out exactly what patients and families need, then offer advice and support as required to form an individualised plan of care.
We are here to provide holistic palliative care at the right time, by the right people in the right place of care.
The Community Hub is a building that will be home to the majority of ACCORD’s Community Orientated Services. Through a collective decision making process the Multi-disciplinary Team will assess every referral and determine the most appropriate individualised and supportive package of care we can offer to meet the patient’s needs at that given time.
