ACCORD has four Clinical Nurse Specialists and a Staff Nurse.
In addition to being Registered Nurses the team hold degrees and have undergone further specialist nurse training in Cancer and Palliative Care.
The role of the Clinical Nurse Specialist is an advisory and supportive one. They work alongside GPs and other Health & Social Care staff to provide care and support to patients and families. They act as a specialist resource to our community colleagues and work in conjunction with other members of the Team.
What You Can Expect
The first contact is a telephone call to introduce you to one of the team, if appropriate you will then be invited to attend the Community Hub at ACCORD to meet one of our Clinical Nurse Specialists. The purpose of this appointment is to meet you and carry out a holistic assessment. From this, a plan of care will be made in agreement with you and your family.
- Assessment and advice on the management of your symptoms - such as pain control, nausea or constipation.
- Emotional support in dealing with anxieties and fears related to illness and treatment.
- Assessment for other ACCORD services, including Inpatient care; Complementary Therapy; ACCORD@Home and Bereavement Services.
- Provide telephone advice for patients and families.Work closely with your existing healthcare team.
